The Benefits Center has just been informed that Champaign Dental Group will
not be participating in the Managed Care Dental program as of June 30.
Therefore, those UI employees with Champaign Dental Group as their provider
have the right to do one of two things:
-- Change to another provider in the same plan (Baytowne Dental Center is now the only provider in Champaign Urbana). To change to Baytowne, call International at (800) 999-1669.
-- Change to Quality Care Dental Plan. To change to the Quality Care Dental Plan, effective July 1, please contact the Benefits Center 3-3111 as soon as possible. No changes will be accepted after July 31.
