Quality Care Health Plan
New identification cards, produced by the Department of Central Management Services (CMS), recently were mailed to the home addresses of all Quality Care Health Plan members. A card was provided for the insured employee along with duplicate cards for any insured dependents age 16 or older.
Members should carry this card with them at all times in case emergency care is needed or to precertify hospital admissions, surgical procedures or mental health services.HMOs
IDs are produced and distributed by the HMO and sent directly to members along with a certificate detailing the plan. You do not have to wait until you receive the card to seek services from your HMO. If you require medical services or a prescription before you receive your ID card, contact your HMO's customer service office and a membership number can be issued. If necessary, the Benefits Center can fax confirmation of your enrollment to the HMO.
Managed Care Dental Plan
ID cards are produced and distributed by the plan administrators directly to members. If you need dental services before your identification card arrives, contact your dental plan's customer service office for assistance.
You do not have to wait until the card arrives to obtain dental services.International Managed Care Dental Plan - Provider Change
As announced in the last issue of "Inside Illinois," effective June 30, the Champaign Dental Group is no longer a managed care plan provider. Any Champaign Dental Group members who have not already elected to switch to the remaining managed care plan provider, Baytowne Dental Center, will automatically be switched to that provider. Employees may switch back to the Quality Care Dental Plan but must contact the Benefits Center by July 31.
Effective July 1, John Hancock Life is the new insurance carrier replacing Metropolitan Life. A new certificate/booklet has been provided by CMS and was recently distributed to the campus addresses of insured employees. This booklet replaces the blue booklet dated Jan. 1, 1995, and should be retained in your personal files as your certificate of insurance. Optional life coverage for yourself or dependents will continue automatically.
On June 24, CMS erroneously sent a notice of termination of dependent coverage for children age 19 and older to several hundred employees. If you received this notice and wish to confirm that your dependent recertification form was received and recorded properly, contact the Benefits Center.
If you would like a copy of your Benefits Statement, which will reflect
your July 1 enrollment changes as well as your current amount of state plan
life insurance, call "Benefits on Call" at 265 UIBC (8422), or
5 8422 from a university phone. Your Benefits Statement can be faxed to
you or mailed to your campus address.
To access "Benefits on Call" you will be asked to enter your Social
Security number. Then you will be asked to secure your record with a personal
identification number (PIN). You determine your own 4-digit PIN, entering
it when prompted by the system. You will then need to use that same PIN
every time you access "Benefits on Call." If you have forgotten
a PIN previously entered, contact the Benefits Center and it can be mailed
to you.
UI employees can enroll in the tax-deferred plans at any time throughout the year. To receive a brochure describing the plan and the investment options, contact the Benefits Center.
Premium changes, which became effective July 1, will be reflected on
the July 21 paycheck for employees paid monthly and on the July 24 paycheck
for those paid biweekly. Insurance premium deductions were not taken out
of the July 10 biweekly payroll - one of the two annual "no deduction"
biweekly payrolls.
If you are enrolled in the Medical Care and/or Dependent Assistance Plans
this year, the first contributions for this plan year will be taken from
the paychecks indicated above. Take a few minutes to review your deductions
and contact the Benefits Center if you have any questions.
